Initial Eval: $100 deposit (only covers cost of evaluation)
Follow-up cosmetic appointment: $100 deposit (is credited to the total of the procedure)
Once deposit is received, the office will contact the patient to proceed with booking. Availability is subject to every schedule.
All deposits aren't refundable, therefore if a change of appointment needs to be made the patient must contact the office 48 labor hours prior the appointment time to be able to reschedule. Deposits are lost if patient misses appointment, and if patient doesn't confirm appointment with 24 labor hours prior. Availability depends on physician.
